[sldev] Good patch practice

I just crated a Wiki page for creating patches.  I took Bryan's
'Good Patch Practise' and added a few extra sections about
the procedure.

I'm not entirely sure if the procedure outlined there this is the
best way to do it, so feel free to comment or change. Also I'm a
non native English speaker and a Wiki n00b, so this will probably
need some polishing.

Also, I guess it should go to the {{OSWikiContribBox}}, I just
have no idea how to do that.  And there are probably other places
to link it from.
