When people are consistently having issues with an interface, maybe the
interface is at fault. The evidence of people having problems with the
interface can be dismissed easily, every time, by saying that the users
are at fault. Microsoft does it, now Linden Lab does it... but blaming
the users is a pretty lame way to do design.

The interface could be done better. If the interface were done better
and a method of protecting people from their mistakes and software
issues were in place, imagine how much time we would save by not having
this conversation repeatedly.
